Keeping seniors in their home is our goal.
As we get older, many of us worry about how long we can continue to live independently. Family Health Care’s in-home screening program can help keep seniors safe and in their own home.

Geriatric Screening and Assessment is for any Newaygo County resident over 65 who lives in their own home or in the home of a relative or community caregiver.

We have special knowledge
The medical diagnosis and treatment of the elderly calls for special knowledge of the aging process and the many factors that contribute to a person’s medical condition.

Our group of professionals meet with seniors and their caregivers in the home to evaluate the senior’s medical, social, mental and functional status.

An assessment involves a physician, a social worker, a speech therapist, and an occupational therapist.

Referrals are accepted from the senior’s physician, community agencies, relatives, caregivers, or the patient.

We help seniors live independently.
In a process that lasts just a few hours, we can make recommendations that can improve the senior’s ability to live independently and to function in a healthy and safe manner. We make referrals immediately. We follow-up as often as needed.

Because we come right to the senior's home:

• We see the home environment first-hand
• There is no need for transportation, nor concerns about leaving the home, forgetting an appointment, or feeling uncomfortable in a clinic
• There is no limit to the amount of time the clinician can visit
