Adult Screening and Prevention Program - Offers health evaluations in the medical, nutrition, cognition, social and sensory needs of older patients in their home. Alzheimer's In-Home Respite - Trained volunteers come to the home as companions, giving caregivers time for themselves. Basic Care - Personal Care Workers - will help with activities of personal living. Friendly Visitors - Trained volunteers make home visits, to visit and talk, or help with non-medical needs. Healthy Beginnings Outreach - We provide an outreach program for young mothers expecting a child, who need assistance and guidance in both Lake and Newaygo Counties. Oakwood Generations - A congregate day care program offering social respite and adult daycare services. Providing activities for individuals suffering with memory disorders. Patient Prescription Assistance Program -
Helps eligible patients obtain low cost or no cost prescriptions, who otherwise would not be able to afford them. Parents as Teachers - Parents have an opportunity to be teachers to their children. We provide the tools and hold monthly meetings to see how our parents are doing. Participants have children ages 0-5. Volunteers - Several of our programs benefit from the efforts of our volunteers.
