Medical Provider Joins Family Health Care’s Big Rapids Clinic
BIG RAPIDS, Michigan – John Kozlowski, FNP-C, has joined the Family Health Care clinic at 15044 220th Avenue
in Big Rapids.
Kozlowski is a board-certified family nurse practitioner. He received his Bachelor of Nursing and Master of Nursing degrees from Chamberlain University in Chicago. He provides a full array of outpatient family medicine services to patients of all ages, including routine exams, chronic disease management, preventative care, and acute illness treatment.
“I’ve always had a strong passion to help people,” Kozlowski said. “I work with my patients to identify the root cause of health concerns and create plans and goals to keep them healthy.”
Kozlowski’s clinical interests include preventative medicine and psychology.

Family Health Care is a Federally Qualified Health Center that provides medical, dental, and behavioral health services at affordable costs. No patient is turned away regardless of insurance status or ability to pay. Most major insurances, including Medicare and Medicaid, are accepted.
