  • Different Terms Apply: When you click on a link to access the Patient Portal, you are leaving familyhealthcare.org and entering a secure environment managed by our third-party vendor.
  • Separation of Services: We do not own, control, or operate the underlying software of the third-party Patient Portal. Your use of the portal is governed by that vendor’s terms of service and our organization’s separate HIPAA Notice of Privacy Practices, not these website Terms.
